Additional Forms

In certain cases, additional documentation may be required to support or manage your claim. The forms below are available to address common scenarios – such as missing ID, acting on behalf of a deceased Class Member, or requesting reconsideration. Please review each form carefully to determine which applies to your situation.

Request for Change in Lawyer or Lawyer Removal

This form should be used if you want to remove a lawyer from your claim. If you want to add a new lawyer, please include their details in Section B.

Lawyer Category 1 Access Authorization Form

Use this form if you completed your Category 1 form independently and now want to allow a lawyer access to it to help with your Category 2 form.

Statutory Declaration Form

If you do not have a valid government ID, or if you need to attest to the nature of your relationship with a deceased Class Member, please have your Guarantor complete a Statutory Declaration form.

Highest Priority Heir Confirmation Form

If you do not have a valid government ID, or if you need to attest to the nature of your relationship with a deceased Class Member, please have your Guarantor complete a Statutory Declaration form.

Lawyer Information Form

If you are a Lawyer completing a Category 1 or Category 2 claim on behalf of a claimant, please complete the Lawyer Information form.

Missing Information Extension Request Form

If you have received a Missing Information Notice but would like to request an extension to the deadline, please complete the Missing Information Extension Request form.

 

Do not use this form unless you have received a Missing Information Notice, otherwise your form will not be considered.

Reconsideration Request Form

If you have received a Denial Notice or a notice of claim approval at a lower category of compensation than you requested and would like to appeal the decision, please complete the Reconsideration form.

Return of Information Form

If you would like to request a copy of the information you provided to the Claims Administrator, please complete the Return of Information form. Please note that copies will be returned, not originals.
