Claim Form Guides: A Step-by-Step Resource for Filing Your Claim

Filing a claim for the Indian Boarding Homes Class Action Settlement doesn’t have to feel overwhelming. These Claim Form Guides were created to make the process easier to understand and to help you complete the right forms with confidence.

 

This blog is your one-stop resource for all the Claim Form Guides, whether you’re applying as an individual, a personal representative, or on behalf of an estate.

 

Eligible claimants may apply for two categories of compensation — Category 1 and Category 2. Each has its own purpose and set of forms. It’s important to begin with Category 1, as this must be submitted before a Category 2 claim can be filed.

Understanding the Two Claim Categories

Category 1 – Claiming Placement

 

The Category 1 Claim Form is used to confirm that you were placed in the Indian Boarding Homes Program.

 

All claimants must complete and submit this form.

 

 

Category 2 – Claiming Abuse

 

The Category 2 Claim Form is optional and is used to claim abuse that occurred while you were placed in the program. You can only submit this form after your Category 1 claim has been filed.

Which Claim Form Should You Choose?

As part of the filing process, you must determine which type of form applies to your situation. There are three claim form options:

 

Individual Claim Form (A1/A2)

 

  • You are a Primary Class Member applying for yourself.
  • You are a lawyer completing the form on behalf of a Primary Class Member.

 

Personal Representative Claim Form (B1/B2)

 

  • You are a Personal Representative appointed to manage the affairs of a Primary Class Member who is under a disability.
  • You are a lawyer completing the form on behalf of a Personal Representative.

 

Estate Claim Form (C1/C2)

 

  • You are applying for a Primary Class Member who has passed away.
  • You are a lawyer completing the form on behalf of an Estate Representative.
